This is meant to keep the report and the outbound HL7 consistent. When you set Enable=true, Harvest will not allow changes to the micro report until a result has been changed on the microbiology worksheet. The relation key in the following OBR's will be in OBR-26. The relation key in the first OBR is with the OBX's, in OBX-4. This allows the host system to produce a result with the correct sensitivities associated with their corresponding organisms. Those will have a relation key that corresponds to the one on the OBX for the ID (isolate) listed under the culture. The sensitivities will be returned as a secondary OBR, with each antibiotic having its own OBX. The end result is to have the outbound HL7 formatted with the culture listed in the first OBR with the results of that as the related OBX fields that each contains a relation key. This made discrete reporting of results necessary. With Meaningful Use II, host systems need to be able to accept these as results instead of comments. The printed report copied this text, and the outbound HL7 sent this out as NTE segments. After allowing the client to manipulate it as desired, Harvest reported the result based on this text field. Previous setups for microbiology took the results out of the worksheet and dropped them into a text field. Discrete Microbiology Results were implemented in Harvest 7.5.
